SMS Program & Opt-In Disclosure

Proof of consent collection for the Copergrine Health & Wellness patient messaging program

1. About This Program

Copergrine LLC(“Copergrine,” “we,” “us”) operates separate SMS programs for its service lines. This page documents how consent is collected for the Copergrine Health & Wellness patient messaging program, operated by Copergrine Health & Wellness, our medical and wellness clinic.

Messages in this program are transactional only. They cover appointment and intake reminders, lab-result notices, billing notices, and login verification codes. We do not send marketing or promotional text messages in this program. Recipients are patients who booked a visit with the clinic and who affirmatively opted in during their own intake.

2. How Patients Opt In

A patient who books a visit with Copergrine Health & Wellness receives a secure, per-patient link to complete their online patient intake before the visit. That link is private to the individual patient and is not a public web page, which is why the opt-in step is reproduced below rather than linked.

The intake form has a step titled “Consents.” On that step the patient sees an SMS / Email Communication (TCPA) checkbox. That checkbox is:

  • UNCHECKED BY DEFAULT.Nothing is pre-selected on the patient’s behalf. The patient must click it themselves to consent.
  • NOT required. The patient can leave it unchecked, submit the intake, and complete their visit normally. Declining does not affect their care in any way.
  • Not a condition of care or purchase. Consent to receive text messages is never bundled with any other agreement, and it is never a condition of being seen, treated, or served.

The full disclosure wording — program description, message frequency, rates, STOP and HELP instructions, and the non-sharing statement — is displayed directly beside the checkbox at the moment of consent, so the patient reads it before opting in. That exact wording is quoted in section 4 below.

4. The Exact Opt-In Wording

This is the consent language displayed beside the checkbox, quoted verbatim as it appears to the patient:

“I agree to receive appointment reminders, lab results, billing notices and other clinic communications from Copergrine Health & Wellness by SMS and email at the contact information above. Message frequency varies by visit. Msg & data rates may apply. Reply STOP to unsubscribe or HELP for help. Consent is not a condition of care or purchase, and I can decline and still be seen. Mobile numbers are never sold, rented or shared with third parties or affiliates for marketing. 5. Record-Keeping

Every opt-in is written to a consent record at the moment the patient submits the intake. Each record stores:

  • the date and time of consent (timestamp);
  • the exact wording that was displayed to the patient;
  • the version identifier of that wording;
  • the channel through which consent was given; and
  • the IP address from which the consent was submitted.

STOP and HELP are honored immediately.A STOP reply ends messaging to that mobile number at once. The opt-out is recorded against that number with its own timestamp — in the patient’s communication preferences and in the audit log — so the opt-out is auditable alongside the opt-in.

  • Consent is not a condition of care or purchase. Patients who decline are seen exactly the same way.
  • Mobile information and messaging consent are not shared with third parties or affiliates for marketing or promotional purposes. Mobile numbers are never sold or rented. They are shared only with the messaging provider that transmits the message on our behalf.
